Patients in whom short duration of therapy may not be possible and in whom duration of therapy should be individualised

Initially inappropriate antibiotic therapy
Severely immunocompromised patients (such as neutropenia or stem cell transplant)
Highly antibiotic-resistant pathogens:
Pseudomonas aeruginosa
 Carbapenem-resistant Acinetobacter spp.
 Carbapenem-resistant Enterobacteriaceae
Second-line antibiotic therapy (e.g. colistin, tigecycline)
